I don't know how to fill out the bank application form.

To see how to fill out the bank application form, you can take a look at the screen capture below.
After you fill-out the form and put your name seal on it, please send it to us.
When we receive the application, it will take about 1-month to process before your payment is automatically withdrawn from your bank account.
Therefore, in the meantime, please make your payment with the enclosed payment form at any convenience store.
 
Moreover, you can also choose the Online Bank linkage service.  you can register your bank account on the Internet. It is more speedy than using the form.
However, it is necessaly that your bank holder's name must be the exact same name as your card holder's name and Furigana. If you are not in this case, you need to register it with the bank application form. (NOT register on the Internet)

【Common Problems Registering Bank Application】
■Problems Related to Name Seal 'HANKO'■
1. Bank Seal Not Registered / Mistake with the Signature
Please make sure that you stamp your name seal or sign your signature on the application that is registered with your bank.
 
2. Name Seal Not Clear
Please be careful to stamp it clearly.
 
 
■Mistake with the Bank Account Information■
Please make sure to check the correct bank name and number and branch name and number since the information could be changed due to bank mergers.
 
■Mistake with Your Name■
Please make sure to write down the exact same name as is written on the cover of your bankbook.
Also, please do not forget to fill in the space for FURIGANA(フリガナ).
e.g.)
Please note: You must use your FULL name. If you have a middle name it will be included on your bankbook, even though it is not included on your Rakuten credit card.
⇒ Please write down your name including the middle name.
 
■Problems Stamping Correction Seal■
If you made any mistakes on the application form, please mark out the mistake by drawing two lines through it. Then, stamp over it using your name seal that is registered with the bank. Please see below.
 
If you would like to request a replacement bank application, you may request one on 楽天e-NAVI「お支払口座の照会・変更」 (Rakuten e-NAVI at "Confirm/Change Bank Account Information") or by contacting the Rakuten Card Customer Call Center.

  1. Press "1" for cardholder.
  2. Press "4" for various changes.
  3. Enter your "card number#" and "date of birth#".
