What should I be careful when I need to send my ID verification documents?

Please note the following two points.
- Please note that the number of submissions varies depending on the type of ID verification document.
- When copying, please be careful that all the necessary items are shown.

- Non-Japanese passports are not accepted.
- Health insurance card
Please fill in the "Symbol/Number",insurance number（記号・番号・保険者番号）, and QR code in black so that we do not see and then submit.

If your current address is not listed on ID verification documents, please check the FAQ below.
2. When copying, please be careful that all the necessary items are shown.
1.Please make a clear copy.
2.If your current address is written on the back side, please copy both sides.
Example
- If your health insurance card has an address column on the back,
please write your current address by handwriting and copy both sides.
